The program I use is XNote Stopwatch. I like it because of its floating nature and global hotkeys so I can turn it on and reset timer by pressing on two keyboard combinations.

Of your list, I have already done the first, second and fifth. They work great.

		<description>This is a really great post.  I need to get back to having a to-do list.  With my illness, I tend to be very forgetful.  It is very frustrating to me, especially when it seems like I have accomplished nothing by the end of the day.
